Since mechanical <br />permit M1811-020 was not issued, I added the work listed under this permit to M1806-047 and voided permit M1811- <br />020. <br />To process the change in contractor, please fill out the attached application with the contractor information, note in the <br />description of work the permit number, that you are taking over from the previous contractor, the date the switch takes <br />effect and note the work that you will be doing. Sign the application and return it to me. I can then update the permit <br />record with the new contractor information and email you a copy of the permit and site card. I am requesting the City of <br />Everett Building Department to switch the contractor on permits M1811-020 and M1806-047 from Interstate <br />Construction license #INTERCL831RD, to TEKAS Fire and Hood Installations LLC, 14611 Ambaum Blvd SW, <br />Burien, WA 98166, license #TEKASFH888JW.
