• aZaZ & ASSOCIATES <br /> GEOTECHNICAL ENGINEERING • ENVIRONMENTAL ENGINEERING <br /> CONSTRUCTION TESTING & INSPECTION <br /> FORENSIC INVESTIGATION <br /> FIELD REPORT <br /> Report No.: 09619024DFR012219-NG <br /> Date: January 22, 2019 Contractor: Sterling Telecomm. <br /> Project No.: 096-19024 Permit No.: B1703-031 <br /> Project: SEA Fleming Inspector: Niccolo Guiao <br /> Project Location: 6320 Evergreen Way Jurisdiction: City of Everett <br /> Krazan PM: JM Weather: Cloudy Temp: 40's F <br /> A Krazan representative visited the project site today for a reinforced concrete inspection. <br /> Upon our arrival today, the contractor had placed the reinforcing steel for the planned CMU wall <br /> footings located at the northwest corner of the Evergreen Professional Center. We verified the <br /> reinforcing steel for size, grade, count, spacing, lap, corner and intersecting bars, and clearances. The <br /> reinforcing steel was adequately secured and supported to prevent significant movement during <br /> concrete placement. In our opinion, the reinforcing steel conforms to approved structural (01/11/17) <br /> and architectural (04/03/17) plans titled, "SEA Fleming" by Camp+ Associates per details 1/S5.0, <br /> 9/S5.0 and 4/A-6. <br /> While at the site, we were requested to evaluate compacted 4-in layer of 5/8-in minus, CSTC only <br /> within the planned footing base. A geotechnical study by Krazan was not performed for this project. <br /> This report does not constitute a footing subgrade evaluation. We evaluated the compacted crushed <br /> gravel using a %2-in diameter steel T-probe rod. The compacted crushed gravel probed up to 2-in <br /> under moderate pressure and appears to be firm and unyielding. <br /> Also, we observed the contractor place approximately 5.5 cubic yards of mix no. 4000464 from <br /> Cadman (Everett Plant). We performed temperature, slump and air content tests of the sampled <br /> concrete. The contractor placed the concrete via truck chute method and was mechanically <br /> consolidated. We cast a set of (5) 4x8 cylinders with concrete breaks scheduled on 1/29/19 and <br /> 2/19/19 with a hold cylinder.
