VVww.SNOH[ .t-RG Environmental Health Division <br /> July 10, 2018 <br /> Nathan Arnold <br /> Subject: Proposed Plan Revision McDonald's, 7920 Evergreen Way, Everett <br /> Dear Mr. Arnold: <br /> After reviewing your plans with the Rules and Regulations of the State Board of Health, and with the <br /> policies of the Snohomish Health District, your plans for McDonald's have been approved. The <br /> conditional approval was based upon the plans submitted June 6 2018. Any changes to the <br /> plans without preapproval from the Health District will void this approval. <br /> A preoperational inspection is required prior to use of menu change and new equipment. At the <br /> time of inspection the construction of the food service establishment must be complete and all <br /> equipment must be in place and in proper operating condition. Incomplete construction or equipment <br /> operation will result in a $185.00 re-inspection fee. Contact the Food Program office a minimum of <br /> one week in advance to schedule an appointment for the preoperational inspection. Please <br /> review the following requirements and conditions of operation before scheduling: <br /> 1. It is understood that the scope of work for this project is the addition of a new combi oven, <br /> staging/holding cabinet and freezer to support a new process for preparing and serving <br /> buttermilk crispy tenders and chicken fillets. A tea brewer, egg cooker, 30" refrigerated <br /> base, and vegetable prep sink will be relocated as a part of this project in order to <br /> accommodate the new equipment. <br /> 2. An indirect waste drain (1 inch vertical air gap, minimum) is required for the combi oven and any <br /> equipment in which food or food contact equipment is placed. Please note that a direct drain to a <br /> grease trap is not considered an indirect drain even if the grease trap is indirectly drained. <br /> 3. All food service equipment, both new and used, must be listed by the National Sanitation <br /> Foundation (NSF) or equivalent for its intended use. Used and new refrigeration must be capable of <br /> holding food at temperature of 41°F or below. <br /> 4. If the establishment continues to be in operation during the remodel, dust barriers must be installed <br /> to ensure that food protection and safety is not compromised. The establishment will be required to <br /> discontinue operations if any disruption in water, electrical, or sewer service occurs, due to the <br /> remodel. Proper handwash stations must be kept accessible and functional during all hours of food <br /> service establishment operation. <br /> 5. The items noted in the submitted documentation stating the procedures that will be used to ensure <br /> food safety during the remodel must be in place during all applicable phases of the remodel. Dust <br /> barriers must also be installed to ensure that food protection and safety is not compromised. <br /> 6. Plumbing must meet state and local codes. <br /> 7. The ventilation system shall be installed and operated to meet applicable building, mechanical, and <br /> fire codes. <br /> Changes or additions to the approved plans or equipment require pre-approval from the Snohomish <br /> Health District prior to implementation of the changes. <br /> A copy of the Snohomish Health District Appeals procedure is available upon request.
