EVERETT <br /> Nom WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2019 through December 31, 2019 <br /> Exhibit A <br /> Program Summary <br /> Organization &Program: Bridgeways—Supported Employment Program <br /> Bridgeways' Supported Employment Program serves unemployed adults living with mental health concerns,who <br /> are Medicaid-eligible, and are typically engaged in community mental health services. <br /> The primary program objectives are to help clients: 1) achieve employment 2) retain employment 3)experience <br /> decreased mental health symptoms that interfere with everyday activities. <br /> Clients have generally experienced symptoms of their mental illness that have presented obstacles to achieving <br /> and retaining employment. Clients are living in very low-income situations, may struggle to maintain positive <br /> relationships and accessing basic needs, including maintaining stable housing. <br /> Supported Employment Program components include vocational assessments,job search assistance, benefits <br /> counseling, resume writing, mock interviews, education on self-care and work attire, interpersonal relationships, <br /> discerning possible on-the-job accommodations, and facilitating connections to other community services and <br /> natural supports. <br /> Supported Employment is driven by the Individual Placement and Support(IPS) model.The IPS model is an <br /> evidence-based practice of supported employment for people living with mental illness that prioritizes rapid job <br /> search,focuses on client preferences, and competitive employment.The IPS model embodies a belief system that <br /> people living with mental health concerns can and should work. Our focus in on getting people to work—not on <br /> training, internships, or pre-work activities. <br /> Collaboration with a client can continue after achieving employment to ensure a successful transition to work--if <br /> the client choses to disclose their mental health concerns to their new employer.With disclosure,the Employment <br /> Case Manager may provide ongoing supports to both the client and the employer—such as discussing possible <br /> accommodations. <br /> The Employment Case Manager(s) is the front line staff, engaging in direct client service. Oversight is provided by <br /> the Director of Clinical Support Services, a Licensed Mental Health Counselor with twenty-three years' experience <br /> providing mental health services,with the last three years in managerial capacity.Additional support,guidance <br /> and assistance with compliance and monitoring is provided by the Manager of Clinical Quality Improvement. <br /> 2019 Human Needs Contract Page 7 <br />
