C. Specific Requirements for Donna Bartleson <br /> 1. Provide leadership and planning for Me and My Shadow & Preschool classes <br /> a) This Professional Services Agreement is effective when the provider receives <br /> a written "Notice To Proceed" with a specific date from the provider's Point of <br /> Contact. <br /> b) Develop curriculum, prepare all materials and activities. <br /> c) Assist and substitute as needed in the preschool classes. <br /> d) Assist with promotion of classes by distributing flyers. <br /> e) Schedule field trips and special days. <br /> f) Monitor and evaluate students' progress. <br /> g) Interact with parents on a daily basis regarding students' progress <br /> h) Monitors students and activity for safety. <br /> i) Maintain attendance records <br /> j) Apply for a Washington State UBI# no later than September 30, 2019 <br /> k) Apply for an Annual City of Everett Business License for 2019 no later than <br /> October 15, 2019 <br /> 2.
