I 2.0 Bidder's Check List 111 The bidder's attention is directed to the following City-provided forms which must be executed in full as required and submitted with the bid: ' 1. Proposal: The lump sum and unit price items must be shown in the space provided. Show unit prices in figures. 2. Proposal Signature Sheet: To be filled in and signed by the Bidder. 1 3. Subcontractor List: To be filled in by the Bidder. 4. Disadvantaged/Women's Business Enterprise Contact Form: To be filled in and signed by the bidder. 5. Non-Collusion Declaration: To be submitted with the bid. 6. Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is accompanied by a certified check. The amount of this bond shall be not less than five percent (5%) of the total amount bid and may be shown in dollars or on a percentage basis. Cash will not be accepted. Failure to complete the aforementioned forms and to submit said forms with the bid may be due cause for rejection of bid. 3.0 Pre-Award Forms The following form is required to be signed and submitted prior to award of Contract: 1. Certification of Compliance with Wage Payment Statutes: To be filled in and signed. This certification is not required to be submitted with the bid proposal and may be submitted after bid opening. The Contract cannot be awarded without this certification. 4.0 Contract Forms The following forms are to be executed after the award of Contract: 1. Contract: This Contract to be executed by the successful bidder in duplicate within twenty(20) calendar days after the award date.
