City of Everett <br /> Washington State STOP Formula Grant Program Renewal Application for FFY 2019 <br /> Coordinated Community Response Team Participation <br /> Agency Submitting this Form: City of Everett Police Department <br /> County: 'Snohomish County I <br /> Date Completed: 9/13/18 <br /> Only one completed form to be submitted for your CCR team. Decide which agency will respond on behalf of all <br /> STOP Grant applicants in your county.The designated agency must submit with their application this completed <br /> CCR form.A copy of the submitted form should be provided to the other STOP Grant applicants in your county <br /> for their files. <br /> A condition of receiving these STOP Formula Grant funds is mandatory attendance at the CCR meetings and <br /> meaningful participation as a member of the county CCR team, which must meet at least quarterly during the <br /> grant period. <br /> While it is a federal requirement that recipients report on their progress in reducing, or reducing the risk of, <br /> domestic violence related homicides, CCR teams should also consider and address services for youth or adult <br /> victims of domestic violence,sexual assault, dating violence,or stalking. During the grant period,CCR teams will <br /> be required to report on their activities. Please keep notes and sign-in sheets from your CCR meetings. <br /> Has the focus and/or have the activities of your CCR team changed since your community planning meeting <br /> last year? If so,why? <br /> No,our focus is the same as last year. <br /> Describe the new focus and/or activities and complete the table below. <br /> I',Domestic violence and sexual assault <br /> What will be done? Who will take the lead?
