COMPLETION DATE REP <br />FAF Delivery 9/19/2011 KGH <br />QTY RATE I AMOUNT <br />installation site, loading surplus soil spoils onto a raised truck bed or <br />platform, provision and/or use of a rebar or wire cage, temporary or <br />permanent casing, jackhammer, de -watering pump, concrete pump, <br />concrete vibrator, or portable generator; building and installing concrete <br />forms; tree, shrub, and all but minor miscellaneous plantings <br />removallrelocation; relocating the foundation hole based upon unforeseen <br />obstructions found one or more feet below the surface; re-routing <br />underground utilities or sprinkler piping; back filling and/or compacting <br />disturbed soil; cutting, removing, and replacing concrete or asphalt; pouring <br />and finishing exposed concrete pads, installing concrete monuments <br />and/or brass plaques; or removing soil spoils, concrete, asphalt, <br />vegetation, old flagpole, etc. from job site. Any or all of these shall <br />generate additional costs against the job, which will be billed out on a time <br />and materials basis only. Our current billing rate for labor is $85 per <br />man-hour and is billed out in 1/6th-hour increments. Unless otherwise <br />quoted, all materials and tool rentals shall be billed out at cost plus 15%. <br />- Any permits and/or underground utility checks shall remain the sole <br />responsibility of the purchaser unless otherwise agreed to in writing by <br />Flags A'Flying, LLC, with all permit and administrative costs associated <br />with this process to be paid by purchaser on a time, with a billing rate of <br />$85 per hour, and materials basis only. PLEASE NOTE: Installation quotes <br />are based on being able to dig the foundation hole and install the ground <br />sleeve on the same date.
