Change Order#2 modifies (1)bid item and adds two (2)new bid items to the <br /> contract as follows: <br /> Modify: Schedule A-Bid Item 5 adds 550 hours of traffic control labor at the unit price of <br /> $49.00/hour,resulting in a$26,950.00 increase to the contract. <br /> Add: Schedule A-Bid Item 62 is added as"Revised HMA Patch Price" at the unit price of <br /> $64.29/ton for 1,204.12 tons,resulting in a$77,412.87 increase to the contract. <br /> Add: Schedule B -Bid Item 36 is added AS "Revised HMA Patch Price"at the unit price of <br /> $64.29/ton for 345.92 tons,resulting in a$22,239.20 increase to the contract. <br /> Change Order#2 also adds 37 calendar days to the contract time due to the many unforeseen conditions <br /> encountered during work on this project. <br /> Change Order#2 is also written to cover final agreements reached between the parties,to allow a final <br /> estimate and cost agreement to achieve project closure and final acceptance. <br /> RECOMMENDATION(Exact action requested of Council): <br /> Authorize the Mayor to sign Change Order#2 with SRV Construction for the Water Main <br /> Improvement"K"and Sewer Main Replacement project for a net increase of$126,602.07,not <br /> including Washington State Sales Tax to the total Contract price. <br /> 1 <br />
