Exhibit"A" <br /> Scope of Services,Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Everett Housing Authority Property Acquisiton <br /> program in a manner satisfactory to the City and consistent with any standards required as a condition of <br /> providing these funds. Such program will include the following activities eligible under the Community <br /> Development Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Parkview Homes XIII project includes property acquisition of 3 single-family homes from the Everett <br /> Housing Authority Property that will serve 10 Persons by providing two (3)three-bedroom homes and one <br /> (1)four-bedroom home, housing a total of ten (10) extremely low-income (at or below thirty percent (30%) <br /> of the area median income (AMI)) individuals. Six (6) individuals will be people who are dually diagnosed <br /> with chronic mental illness (CMI) and intellectual and developmental disability (IDDs) and four(4)will be <br /> people with IDDs who may or may not be diagnosed with CMI. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Tenants will receive 24/7 in-home support services funded by the Developmental Disabilities <br /> Administration (DDA).The proposed homes will be in Everett and provide affordable, accessible housing for <br /> at least 40 years for extremely low-income individuals with developmental disabilities and/or chronic <br /> mental illness. <br /> B. National Objectives <br /> The Subrecipient certifies that the activities carried out with funds provided under this Agreement will meet <br /> the following CDBG National Objective, as identified by the City of Everett for priority in the 2019 Program <br /> Year,"Benefit low/moderate income persons"-as defined in 24 CFR Part 570.208. <br /> Appropriate subcategory under the National Objective: <br /> Housing <br /> 14G - Rehabilitation:Acquisition 24 CFR 570.202(b)(1)or 42 USC 5305(a)(1)only. <br />
