( See attachment 4- <br />12-13) The design requirements for interior slab on grade is 3000 psi as per [he noles on 51.01. 7he Cylinder brea4 tests <br />show tha; the 7 day results exceeds the 3000 psi requirement found in the structural notes. It is uniortunate that a <br />mista4e was made when ordering the concrete and then not detected at time of placement. I have talked to Joe Zlab <br />hvice on the matter, as the representauves of the ownersh�p �ve feel comfor[able in going forward with the mncrete <br />that has been placed. As long as no building codes has heen breached �ve are accepting the concrete as is. I( you see any <br />further concems about this, please �et us know at your earliest convenience.. You are correct in noting ihai concrete <br />mix des�gn tt025560 was prev�ousiy submrtted and rejected �in Januaryl due [o insufficient data Per your descripLon, rt <br />is our unders[and�ng that the slab-on-grade �nfx »025506 (which was not approved) was substituted a� somc slab on <br />grade areas. As you have indicated, the 7-day concre[e brealc strengths of mix «025506 are greater than 3000 psi, th� <br />required 28-day stren�th for the slab-on-grade. Be advised that we do approve of this substitution. To prevent <br />potent�al expensive corrective measures, we highly recommend that any future substrtutions be submitted for our <br />review prior to implemen[ation. <br />Pa�e 8(Test Report a31): There �s re`erence to concre[e mix type 025506 at some of the slab-on-grade areas.
