fi <br /> AMENDMENT NO. 1 <br /> CONTRACT FOR BASIC LIFE SUPPORT AMBULANCE SERVICES <br /> BETWEEN TH H; CITY OF EVERETT <br /> AND NW AMBULANCE <br /> This Amendment No. 1 is by and between the City of Everett, a municipal <br /> corporation under the laws of the State of Washington ("City") and Platinum Nine <br /> Holdings, LLC dba NW Ambulance, a Washington limited liability company <br /> ("Contractor"). <br /> RECITALS <br /> A. The City and Contractor are parties to the Contract for Basic Life Support <br /> Ambulance Services dated July 1,2016 (the"Agreement"). <br /> B. The City and the Contractor desire to amend the Agreement for the purpose <br /> of making certain revisions to the monthly payments and rates. The City and Contractor <br /> also desire to extend the term of the Agreement,so that the term expires August 31,2024. <br /> AGREEMENT <br /> The City and Contractor agree as follows: <br /> 1. Section 11.2.1 of the Agreement is amended as follows,with strikeouts deleted and <br /> underlines added: <br /> 11.2.1 Monthly Payment: The Contractor shall make an annual payment of <br /> $158,167.01 $39,541.76 payable in equal monthly payments of$13,180.59 <br /> $3,295.14 to the City to cover the City's costs of administering the Contract <br /> and providing resources to support BLS transport. Such costs include and <br /> are not limited to: <br /> Subsections 11.2.1.1-11.2.1.4 remain unchanged. This revision is effective <br /> December 1,2019,so that Contractor's December 2019 payment will be$3,295.14. <br /> This revision does not affect payments due prior to December 2019. <br /> 2. The first paragraph of Section 11.3.2 of the Agreement is amended as follows,with <br /> strikeouts deleted and underlines added: <br /> 11.3.2 Rates: .- ... . <br /> - •• - ' - . The Contractor's rates shall be BLS - <br /> Non-Emergency Base Rate per Transport: $950.00. Mileage Rateper mile <br /> $20.15 BLS -Emergency Base Rate per Transport: $950.00 Mileage Rate <br /> per mile$20.15 Critical Care(Nurse)Base Rate per Transport: $2,105.88 <br /> Mileage Rate.per mile$20.15. Contractor shall not separately charge, <br /> separately invoice, or separately itemize on invoices disposable supplies, <br /> equipment usage, or recoupment of payments made including liquidated <br /> • damages to the City pursuant to Sections 11.2 and 11.4. <br /> 40 <br />
