Exhibit A <br /> Diking District 5 <br /> Dike Repairs and Improvements Project <br /> Design for Dike Repairs Under 1-5 and East of 1-5 <br /> (Approximate Dike Stations DD5-A-325+00 to DD5-A-343+00) <br /> (Amendment 2) <br /> Otak Project No. 32758 <br /> October 2019 <br /> Background Information <br /> This Diking District 5 Dike Repair project is located on Smith Island, northeast of the City of Everett(the <br /> City), within the Snohomish River Estuary. There is a system of dikes around Smith Island that protect the <br /> island from much of the flooding that can occur due to tides and river flows. Portions of the dike system <br /> along Union Slough have eroded, and some areas require repair to meet U.S. Army Corps of Engineers' <br /> standards for the PL 84-99 program. <br /> Phase 1 of the project included survey and mapping, geotechnical investigations, environmental studies, <br /> and engineering analyses to provide DD5 with a comparison of alternatives for the repairs. The original <br /> Phase 2 amendment contract provided final design of dike repairs west of Interstate-5 (1-5)to SR 529 <br /> (Approx. Dike Stationing DD5-A 343+00 to DD5-A 376+50) This amendment provides preliminary and <br /> final design for repairs to the dike from the County's new setback dike (approximate Dike Stationing DD5- <br /> A-325+00), to just west of 1-5 (DD5-A-343+00), including the dike underneath 1-5. The dike stationing <br /> referenced in this scope of services is the dike stationing approved by the DD5 in October 2019. <br /> Project Approach <br /> Otak with its subconsultant, Cornforth Consultants, Inc. (CCI), (the Otak team)will provide additional <br /> survey, geotechnical recommendations, final design and construction documents, for the additional areas <br /> of dike under 1-5, and east of 1-5 to the County's setback dike. The following tasks are added in this <br /> amendment to Phase 2 of the DD5 Dike Repairs Project. <br /> Task 1 — Project Management and Coordination (supplement to original Phase 2 contract <br /> amendment) <br /> Task 8— Dike Repair Under 1-5 Final Design (new task) <br /> Task 9— Upstream Dike Repair Final Design (new task) <br /> The Scope of Services are anticipated to commence in November of 2019, with completion of final design <br /> anticipated for August of 2020. <br /> Task 1 - Project Management and Coordination <br /> 1.1 Monthly Project Status Report and Invoices <br /> Brief project status reports will be submitted to the City that summarize tasks accomplished during the <br /> month, tasks projected for the next month, information or actions necessary to maintain project <br /> DD5 Dike Repairs and Improvements Project 1 <br /> otak <br /> K:\project\32700\32758\Contract\Proposal_19_0317\Scope for Phase 2-DD5 dike repairs Under 1-5 and East of 1-5_19_1014.docx <br />
