C. Specific Requirements for Janel Cziske <br /> 1. Provide leadership and planning for The Preschool at Forest Park <br /> a) This Professional Services Agreement is effective when the provider receives a <br /> written "Notice To Proceed" with a specific date from the provider's Point of <br /> Contact. <br /> b) Assist with developing curriculum, prepare all materials and activities <br /> c) Assist with promotion of classes by distributing flyers. <br /> d) Assist with scheduling field trips and special days <br /> e) Monitor and evaluate students' progress. <br /> f) Interact with parents on a daily basis regarding students' progress <br /> g) Monitors students and activity for safety. <br /> h) Fill-in as a substitute for the other Preschool instructors as necessary <br /> i) Obtain a City of Everett Business License for 2020, no later than January 31, <br /> 2020 <br /> 2. Dates and times - Days of the week are determined on the program attendance <br /> Position Dates Day Time Location <br /> Assistant Jan-Dec Mon/Wed 9- 11:30am Preschool Room <br /> Preschool 2020 and/or 12:30-3pm <br /> teacher Tu/Th 9:30-11:30am <br /> 12:30-2:30pm <br /> 3. Plus additional classes/programs as agreed upon by Program Supervisor and <br /> contractor. <br /> 4. The City retains the right to cancel any program which does not meet minimum <br /> registration requirements. <br />
