C. Specific Requirements for Camp Fire Snohomish County <br /> 1. Provide the following for all classes taught <br /> a) This Professional Services Agreement is effective when the provider receives <br /> a written "Notice To Proceed" with a specific date from the provider's Point of <br /> Contact. <br /> b) Plan and implement Self-Reliance classes for children 7-14 years old <br /> c) Maintain attendance records <br /> d) Assist with promotion of classes by distributing flyers. <br /> e) Monitor and evaluate student progress. <br /> f) Maintain and submit course evaluations. <br /> g) Monitors students and activity for safety. <br /> h) Prepare class materials and handouts for each class <br /> i) Renew Annual City of Everett Business License for 2020, no later than <br /> January 31, 2020 <br /> 2. Dates and Times of Program Currently Scheduled <br /> Class Dates Day Time Ages Min/Max Location <br /> Baby Sitting Feb 22-29, Sa 9am- 10-14 8/15 Preschool <br /> Basics 2020 1pm Room <br /> 3. Plus additional classes/programs as agreed upon by Program Supervisor and <br /> Service Provider as detailed in the Quarterly Program Guide. <br /> 4. The City retains the right to cancel any program that does not meet minimum <br /> registration requirements. <br />
