C. Specific Requirements <br /> 1. Will provide clinics with instruction for all registered participants. <br /> a) All Skyhawks staff members will pass background checks conducted by <br /> Skyhawks Sports Academy. <br /> b) Skyhawks will keep attendance roster provided by the Point of Conduct and will <br /> return completed attendance roster to the Point of Conduct after each session. <br /> c) Provide equipment lists and agendas as needed. <br /> d) Skyhawks and the lead counselor will be available at all times during camp hours <br /> via cell phone provided by Skyhawks or by use of a personal cell phone. <br /> e) Skyhawks will have a lead counselor present at all times. <br /> f) Develop curriculum to instruct outdoor activities. <br /> g) Monitor and evaluate student's progress. <br /> h) Maintain and submit course evaluations. <br /> i) Maintain course attendance sheets. <br /> j) Monitors students and activity for safety. <br /> k) Will maintain a 12:1 camp to counselor ratio. 14:1 for Soccer and Lacrosse <br /> I) Will maintain all facilities used by following facility rules and reporting any <br /> damage before, during or after facility use. <br /> m) Skyhawks will report any problems to the Point of Conduct within 24 hrs of <br /> occurrence. <br /> n) Skyhawks will not allow "drop-in" use at any camp event. All participants must <br /> use proper registration procedures to participate. All participants must be paid in <br /> full prior to participation. <br /> o) Skyhawks will have emergency contacts and reachable phone numbers for all <br /> participants. <br /> 2. Dates and times — Refer to Exhibit D-Registration Flyer <br /> 3. Plus additional classes/programs as agreed upon by Program Supervisor and <br /> Service Provider as detailed in the Quarterly Guide <br /> 4. The City retains the right to cancel any program which does not meet minimum <br /> registration requirements. <br /> 5. Payment <br /> Base Registration Fee Payment to Service <br /> Provider (75%) <br /> Full Day Camp $179 $134.25 <br /> Half Day Camp $145 $108.75 <br /> Saturday Clinics $90-$120 $67.5-$90 <br />
