PWUU11-029 <br /> PW0011-030 <br /> PW0011-028 �,�,� <br /> PW0012-001 �y�.�,�c����c.p-f�^`�'� o�^v'� <br /> PermitNumber � � � ��� �5��� <br /> � Pa'�^�°' i -s-o� <br /> CITY OF EVERETT PUBLIC WORKS <br /> REVOCABLE GENERALPERMIT <br /> FOR ACTNTI'Y WITHIN THE PUBLIC RIGHT-OF-WAY <br /> The City of Everett hereby grants a non-exclusive revocable general pemiit to Advanced TelCom, Inc. dba <br /> Advanced Te1Com Group, a Delaware corporation, and its respective successors and assigns, whose address is <br /> 110 Stony Point Road,2"d Floor, Santa Rosa, CA 95401 and 1111 Comwall Ave., #A-1, Bellingham, WA <br /> 98225,hereinafter refeaed to as "the Permittee,"pending adoption and approval of a franchise ordinance. <br /> Permit <br /> The Pertnittee is nereby authorized to install, commission and test all wires, lines, cables, conduits, equipment, <br /> and supporting structures ("Facilities"), located in the City's right-of-way, to be utilized by the Permittee in the <br /> operation of its telecommunication system, subject to the tertns and conditions of this Petmit. <br /> Term <br /> This Permit is effective commencing on the 3 day of.1an., 2001, and terminates on the sooner of one yeaz <br /> from the effective date, the acceptance by the Permittee of a franchise ordinance approved by City Council, <br /> upon ninety days' written notice by the City, or as othenvise provi3ed Uy this Permit. <br /> Conditions <br /> This Permit is subject to the following conditions: <br /> l. This permit only authorizes those activities described in Exhibit A. It does not authorize major activities <br /> which will require a separate permit. <br /> 2. The Permittee must pay the City a permit processing and inspection fee in the amount of Eight Hundred <br /> and Eighty-Five and No/100 Dollars($885.00) at the time ofpemtit application. <br /> 3. For each sepazate use of the right-of-way under this permit and prior to commencing any work on the <br /> right-of-way under this permit, the Permittee shall: <br /> a. Give the City the nutice which shall include both the permit uumber and tlre street address <br /> nearest to the proposed�vork site; and <br /> b. The Permittee will telephone the City's Public Works Department at least one(I)business day <br /> (24 hours)prior to commencing work. <br /> c. All activity on the right-of-way shall be included on the monthly list described in paragraph two <br /> which is provided to the City. <br /> 4. The Permittee agrees to, and]iereby does release, indemnify and promise to defend and save hazmless <br /> the City, its officers, agents, and employees from any and all claims for damages, demands or causes of <br /> action, whether to persons or property, that in any manner directly or indirectly arise out of the <br />
