Project: Cleveland Avenue Sewer System Replacement& Storm Water Separation <br /> Change Order No. 1 <br /> Change Order Effective Date: November 1, 2019 <br /> CONTRACTOR ' <br /> l� <br /> By Date: <br /> ct <br /> Officer <br /> Exhibit A—Description of Changed Work <br /> This change order#1 adds one new item to both schedules A&B and modifies one existing bid item <br /> on both schedules A&B of the Contract as follows: <br /> Schedule A: <br /> Modify existing bid item#11 -Foundation Material,by deleting 2,130 tons of foundation material,at <br /> the unit bid price of$22.00 per ton,resulting in a $46,860.00 credit to the contract price. <br /> Add new item#100 - Concrete Roadway Trench Patch,by adding 436.50 square yards of concrete <br /> roadway trench patch, at the agreed to unit price of$72.50 per square yard, resulting in a$31,646.25 <br /> contract price increase. <br /> Schedule B: <br /> Modify existing bid item#11 -Foundation Material, by deleting 2,440 tons of foundation material, at <br /> the unit bid price of$22.00 per ton,resulting in a$53,680.00 credit on the contract price. <br /> Add new item#100- Concrete Roadway Trench Patch,by adding 556.00 square yards of concrete <br /> roadway trench patch, at the agreed to unit price of$72.50 per square yard, resulting in a$40,310.00 <br /> contract price increase. <br /> The net result of adding one new item#100 to both schedules A&B and modifying one existing bid <br /> item#11 on both schedules A&B of the contract, results in a contract price reduction of($28,583.75). <br /> JUSTIFICATION: <br /> Existing bid item#11, for Foundation Material,were not needed on schedules A&B of the project. <br /> New items#100--Concrete Roadway Trench Patch,were needed on both schedules A&B of the <br /> project as existing roadway concrete panels were discovered underlaying existing asphalt on Summit <br /> and Everett Avenues,which were not accounted for in the contract documents. <br /> CONTRACT TIME: <br /> No additional days are added to the contract time as a result of change order#1. <br /> Page 3 of 3 <br />
