5109 Seahurst Short-Mat Demo Permit Narrative <br /> To:City of Everett Building Dept <br /> From:Jonathan Shelton, home owner of 5109 Seahurst Ave, Everett,WA 98203 <br /> Project Location:5109 Seahurst Ave, Everett,WA 98203 <br /> Project Number with COE Building Dept.: 18-037 <br /> Scope: Demolition of a 250 SF carport roof, roof framing,carport concrete slab on grade,wood deck, <br /> concrete patio slab on grade and small section of wood fencing.This work will occur before future new <br /> construction of a proposed garage and short-plat are approved by the COE planning department. <br /> On 11/6/2018 I contacted the Puget Sound Clean Air Agency and spoke with inspection manager Rick <br /> Hess(206-689-4029 ext 4029) regarding the details of my demolition project. <br /> PSCAA's definition of DEMOLITION is a structure that will be uninhabitable.A RENOVATION is <br /> remodeling or modifications done by a home owner who still occupies the building. I own and occupy <br /> the existing home located at 5109 Seahurst Ave. <br /> PSCAA renovation work requires the home owner to do the following: <br /> 1. Asbestos Survey <br /> a. An AHERA certified inspection not required. Home owner can take samples of suspect <br /> materials. <br /> b. Survey Report. <br /> c. Samples <br /> i. Two each from different locations of suspect materials. <br /> ii. Home owner can take samples to testing lab. <br /> If samples contain no asbestos,then results are not required to be submitted to PSCAA, but are required <br /> to be retained on site until demolition is finished. No report on site will result in a citation if PSCAA <br /> randomly inspects the jobsite. <br /> PSCAA will not give a home owner a form to submit to the COE Building Department, because no form is <br /> required for RENOVATION. <br /> NVL Lab asbestos testing was NEGATIVE for all samples of planned assemblies to be demolished for <br /> renovation.
