D. Levels of Accomplishment- Goals and Performance Measures In addition to the normal administrative services required as part of this Agreement, the Subrecipient agrees to provide the following levels of program services: Activity Units of Service* per Month Total Units/Year Leam &Earn A Bike Class Avg of 3 class sessions 5 classes per month Total People or Households to be served Total unduplicated Everett Residents or Households to be served 30 people 30 residents *Note: Provide definition of Units of Service, which may include such measures as units rehabbed, persons or households assisted, or meals served, here. If this is a construction project, please explain here how the improvements will be used and how they benefit the community, what the target population is for the project, etc. The Subrecipient agrees to track outcome performance measures as required by the U.S. Department of Housing and Urban Development (HUD) and the City. The Subrecipient will report the outcomes to the City on the following schedule: Performance Period Report Due October 1, 2019—December 31, 2019 January 31, 2020 January 1, 2020—March 1, 2020 April 30, 2020 April 1, 2020— June 30, 2020 July 31, 2020 Reporting forms must be in the format acceptable to the City of Everett. Exhibit "E" shall be used for all reporting requirements under this agreement. E. Staffing [List of staff and time commitments to be allocated to the activity specified in l.A. above] Program Instructor: 210 hours total, 42 hours per class - 6 sessions of 3 hours each plus 2 hours of prep time each and 2 hours of followup assistance per student (6 students) Executive Director: 32 total hours of program management and reporting (8 hours per quarter)
