As of January 1, 1999 a�iy food service establishment advertising the availabiliry of tobacco or tobacco <br /> products is subject to Snohomish Health District Boazd of Health Resoludon 98-24. This resolution <br /> contvns re;ulations governing outdoor advertisement of tobacco and tobacco products. Further <br /> information on Resolution 98-24 is or, the enclosed information sheet. <br /> 2. An additional handwash sink i� :�q::ired at the panni trill end of the NW Deli area. <br /> 3. A horizontal sepazation of at least l6 inches or a vertical partitior. 16 inches in height is required <br /> between the food prepazation sink and all other sinks. <br /> 4. All food servic�equipment must be listed by the National Sanitation Foundation(NSF) for its intended <br /> use. <br /> 5. If proposed new equipment is the same as that being installed at the remodel of the QFC#355 at 17525 <br /> Hwy. 99, Lynnwood, an equipment list submittal is not needed. If any of the equipment is different, <br /> then an equipment list with manufac[urers nantes and mode! numbers must be subnutted prior to the <br /> preoperationalinspection. <br /> Note: customer self service dispensers must be of a design so as to prevent direct hand contact of the food <br /> by the customer. The dispenser must:il�o be easily cleanable and be constructed of food grade materials. <br /> A preoperaponal inspection is requi:ed prior to opening for business. At the time of inspection the <br /> construction of the food service eseablishment must be complete and all equipment must be in place. <br /> Incompiete construction may resu.it in a $65.00 reinspection fee. Please contact the Food Program <br /> office a minimum of one wee� in advance to schedule an appointment. This will ensure <br /> compliance with the Rules and Regulations of the State Board of Health for Food Service Sanitation. <br /> If there are any significant changes or additions to your layout or equipment, [he Snohomish Health <br /> District must be notified. <br /> Please contacr me if you hav:: any questions.
