5 CITY OF EVERETT REQUEST FOR QUALIFICATIONS (RFQ) FOR SURVEYING SERVICES The City of f Everett is requesting a Statement of Qualifications (SOQ) for surveying services. The work shall include design surveys, construction staking and miscellaneous mapping and control surveying services. Most work done for the City shall be submitted in "DWG" file format, AutoCAD version 2013 or later. Format, layering, and symbology shall conform with the "Surveying and CAD Standards for the City of Everett," and be submitted in a Project Survey Binder as outlined in the standards. Finns shall be capable of providing point files in Civil 3D or in Land XML or other format, and TIN files with breaklines — compatible with AutoDesk Civil 3D. Based on the qualification found in the SOQ, firms will be selected and placed on a list by which work will be distributed. The list will be placed on file for use by all City departments on an"as needed" basis. The City will negotiate a two-year common contract that will include a rate schedule with a second year inflation adjustment for use with each of the firms selected. Firms selected will be required to include the City as a named insured on any vehicle liability insurance policies covering vehicles used by the survey crews on City projects. Interested firms should submit their qualifications and include the following information: 1. Background and experience, complete with references. 2. Qualifications of key personnel. 3. Capability to provide drawing files in AutoCAD 2013, or later version. 4. Ability to provide in-house GPS services. 5. 2014 survey rate schedule used for the general public. The rate schedule should include(per hour): Field Work Personnel 1. Two-person survey crew 2. Three-person survey crew 3. Project Surveyor(LSIT) Equipment 1. GPS unit and accessories 2. Theodolite, Total Station, Digital level and accessories Office Work 1. Professional Engineer(P.E.) 2. Senior PLS 3. Junior PLS 4. CAD Technician 5. Accounting/Office Support 10
