With the addition of the followin� ,:ne plans <br />aze approved. <br />1. The Health District operating pernnit application process must be completed prior to opening <br />for business. <br />2. An indirect waste is required for the food preparation sinl:, mechanical dishwasher, ice <br />machine, running water dipper well, and equipment in which food is placed. <br />3. A horizontal separation of at least 16 inches or a vertical partition 16 inches in height is <br />required between the food preparation sink and all other sinks. <br />d. Extra wall protection is required on walls behind sinks and food preparation tables. A 16- <br />inch high backsplash of plastic laminate, fiberglass reinforced plastic or equal is acceptable. <br />Floor to ceiling protection is required on wall behind dishwashers. Plastic coated hardboard <br />is not acceptable. <br />5. The ceiling above the food preparation areas must be nonabsorbent, smooth, and easily <br />cleanable. <br />6. All light fixtures in food preparatiun anil storage areas must be provided with covers or <br />shatterproof bulbs. • <br />7. Plumbin; must meet state and local codes. <br />8. The ventilation system shall be installed and operated to meet applicable building, <br />mechanical, and fire codes. <br />A pre-operational inspection is required prior to opening for basiness. At the time of <br />inspection the construction of the food service establishment must be complete and all equipment <br />must be in place. Incomplete construction may result in a$130.00 reinspection fee. Contact the <br />Food Program oftice a minimum oF one week in advance Yo schedule an appointment. This <br />will ensure compliance with the Rules_ and Reeulations of the State Board of Health for Food <br />Sei vice Sanitation. <br />
