EXHIBIT A <br /> SCOPE OF WORK <br /> The purpose of this contract is a hazardous materials assessment for the Everett Municipal Building, <br /> located at 2930 Wetmore Avenue, Everett WA. The areas covered by this assessment are as follows: <br /> 1 1St Floor—City Clerk Security Improvements <br /> 2. 4th Floor—Prosecutors Carpet& Paint <br /> 3 9th Floor—Accounting Carpet& Paint <br /> 4 10th Floor—Administration Northeast Corner Remodel <br /> Asbestos Survey: <br /> The scope for the certified Asbestos Good Faith Inspection includes,a current accredited building <br /> inspectors'visual inspection, bulk material sampling for asbestos&lead paint chips, minor repair of <br /> sampled areas,sample analysis, certified reports,a sample location drawing& photographs, and a final <br /> written interpretive report. The standard turnaround time(TAT)for analysis is typically 10 days,and the <br /> final report would typically be delivered 14 business days following receipt of the analytical reports. The <br /> final report could be delivered in four to six weeks from the notice to proceed, NTP,assuming there are <br /> no access delays for initial and follow-up sampling. The purpose of conducting the Asbestos Good Faith <br /> Inspection is for worker protection during the proposed remodeling and renovation identified at the <br /> subject site. <br /> Asbestos Survey Deliverables: <br /> The following items shall be delivered to the City of Everett at the completion of the survey. <br /> 1. AGFI—2 hard copies in separate 3 ring binders <br /> 2. AGFI—Electronic Copy—PDF version <br /> 3. Lead Paint Chip Survey—include results with AGFI <br /> Optional Services: <br /> Pending results of the Asbestos Good Faith Inspection the Environmental Consultant may provide an <br /> environmental submittals checklist, technical review of submittals, and on-call construction surveillance <br /> and monitoring. <br /> Additional Services(Project Contingency): <br /> Additional Services that may be needed based upon any issues that surface during the development of <br /> the project as approved only in writing and signed by an authorized City representative. <br /> Staff <br /> Alternative Environmental Technologies shall execute the work with the staff as follows: <br /> 1.
