Senior Center— Facility Monitor <br /> Exhibit A - Scope of Work <br /> General Requirements <br /> 1. CONTACT. Work with your primary Point of Contact Eric Wollan (425) 257-7082. for any program <br /> questions, changes, accidents or needs. In the event your primary Point of Contact is not <br /> available, your secondary Point of Contact is: Bob Dvorak(425) 257-8780. <br /> 2. ACCIDENTS. All injuries, regardless of apparent severity are to be reported immediately. <br /> Accident reports are to be completed by facility monitor and senior center staff based on <br /> information taken by Facility monitor. Facility monitor will work with their point of contact in any <br /> accident. <br /> 3. SCHEDULE. Facility monitor shall plan to arrive 30 minutes prior to the facility rental and stay <br /> until all guests/renters have left the building. Upon arrival of renters, facility monitor will greet <br /> the renters, show them the room as set up and ensure that they have all AV equipment/other <br /> equipment needed for their event. <br /> a.A checklist will be provided which outlines AV needs. <br /> b.A cleaning checklist will also be provided for the monitor to use at the end of the rental <br /> period. <br /> c. 20-25 minutes prior to the end of the rental, the monitor will remind the renter about the <br /> time and begin reviewing the checklist for/remind renters of cleaning expectations. <br /> d.After all guests have left, the monitor will turn off all lights and use their code to set the <br /> alarm before exiting. <br /> 4. COMPENSATION. Facility monitor will bill the Senior Center at the end of each month. Invoice <br /> must show date and times worked at $13.50 per hour. Facility monitor will not be <br /> compensated for rentals that have been refunded or credited back to a patron. No money shall <br /> take place at the time of the rental, including the giving or receiving of tips. <br /> 5. PAYMENT. A signed invoice from Facility Monitor is needed to process payment. All services for a <br /> calendar year must be invoiced no later than January 5th of the next calendar year. <br /> 6. CANCELLATIONS. Facility Monitors are not to make decisions regarding rental cancellations, <br /> etc., without approval from the program coordinator. If Facility Monitors needs to cancel <br /> appointments, please call the Point of Contact. If the supervisor is not available, please call <br /> the staff counter at the Carl Gipson Senior Center. <br /> 7. TELEPHONE USE. City telephones are not to be used for personal calls during the rental <br /> they are monitoring. Personal use of the phones is limited to emergencies or class-related <br /> urgent needs. <br /> 8. CLERICAL ASSISTANCE. Facility Monitors who need clerical assistance (i.e. mailing, <br /> copying, or phone calling)from the office staff must submit the request to the Point of Contact <br /> at least one week prior to your due date. <br /> 9. CHILD CARE. Children and grandchildren of Facility Monitors are not allowed to be present in <br /> the senior center while the Facility Monitor is monitoring. It is the responsibility of the Facility <br /> Monitor to arrange adequate child care. Adult friends and family need authorization prior to the <br /> rental in order to accompany the facility monitor. <br /> 1 <br />
