EVERETT <br /> IMO WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2020 through December 31, 2020 <br /> Exhibit A <br /> Program Summary <br /> Organization&Program: Catholic Community Services:Volunteer Chore Services <br /> The Volunteer Coordinator secures volunteers who serve elders and adults who have functional disabilities,to provide the <br /> support they need to live with dignity in their own homes and to provide access to health and social services that will help <br /> them remain an active part of our community. <br /> Volunteer Chore Services has 8 components: <br /> 1) Client Intake <br /> 2) Client in-home assessment <br /> 3) Volunteer Recruitment <br /> 4) Volunteer Orientation <br /> 5) Volunteer and client match <br /> 6) Volunteer training <br /> 7) Client follow-up <br /> 8) Volunteer follow-up concerning match between client and volunteer. <br /> Client referrals come from various sources including D.S.H.S,Senior Services,doctor's offices,hospital social workers, <br /> insurance providers,relatives,and neighbors,and self-referrals.Volunteers provide services for clients and assist with <br /> activities of daily living.Their interaction with the clients minimizes safety concerns and the isolation which is frequently <br /> experienced by elders and bridges the service gap for persons ineligible for paid chore services. <br /> 7 <br />
