EVERETT <br /> NM WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2020 through December 31, 2020 <br /> Exhibit A <br /> Program Summary <br /> Organization&Program: Volunteers of America:Housing Crisis Intervention. Mediation,and Homelessness <br /> Prevention <br /> The Housing Crisis Intervention,Mediation,and Homelessness Prevention Program will serve Everett residents experiencing <br /> housing instability that would lead to homelessness without intervention.Eligible program participants will be seniors(60+) <br /> and individuals with disabilities,who are very low income(0-30%median)and low income(31-50%median). <br /> This program will resolve housing crisis,conflicts and disagreements to prevent evictions and displacements or provide <br /> relocation assistance and support securing new housing to avoid homelessness. <br /> The main objectives: <br /> -Reduce caseload of Courts by resolving housing issues and preventing eviction filing <br /> -Provide Housing Stability Navigation/Case management:referrals to mental health,chemical dependency,gambling, <br /> hoarding,and family counseling;explore potential income increase through employment support(WorkSource employment <br /> navigation or Ticket to Work referral for individuals with disabilities),apply for cash and food benefits, Social Security or <br /> disability benefits,and money management education <br /> -Provide short term fmancial assistance(rent arrears,security deposit,rental application fee,moving expenses,transportation <br /> assistance)to avoid homelessness <br /> -Emergency hotel vouchers for most vulnerable households while transitioning into new housing <br /> -Respond to Client Concerns and provide Information regarding Residential Landlord-Tenant Act,Manufactured/Mobile <br /> Home Landlord Tenant Act,and 2019 Changes to Washington's Landlord/Tenant and Eviction Laws <br /> -Provide Conflict Coaching,Conciliation,Negotiation or Mediation to stabilize housing(including payment plans or <br /> behavioral contracts to ensure compliance with lease agreements) <br /> The Program Structure: <br /> 1)Receive referrals from 211,Coordinated Entry System,or Landlord-Tenant Phone Line <br /> 2)Program Manager assigns eligible cases within 24 hours to Homelessness Prevention Navigators <br /> 3)Navigators conduct phone interview and in person meeting to identify barriers to housing stability and retention and create <br /> Individualized Housing Stability Plan <br /> 4)Navigators work with client to resolve housing instability for up to 90 days(communication/conciliation/negotiation with <br /> landlord,case management meetings,completing action steps,administering fmancial assistance or emergency hotel <br /> vouchers,housing search and placement,and referrals to other providers as needed) <br /> 5)Exit interview/Data Entry/Case Closure <br /> Meetings will happen at the DRC office,at the housing site,if property managers involved,and to accommodate clients with <br /> disabilities in client homes. <br /> Outputs/Outcomes: <br /> The program will serve 40 eligible households.Each eligible household would have at least one senior or at least one person <br /> with a disability,receiving short term disability income,Aged Blind Disabled assistance, Social Security Disability,or <br /> Supplemental Security Income.70%of the households will be very low income,and 30%low income. <br /> 85%of served households will stabilize their housing crisis prior to program exit. <br /> 7 <br />
