APPENDIX A <br /> SCOPE OF WORK <br /> Service Provider will conduct a mixed-mode survey of adults in Everett, WA. The number <br /> of surveyed adults will be at least 400. The duration of each survey will be 15 minutes. All <br /> survey questions and survey methods must be pre-approved by the City. Scope of work includes <br /> questionnaire design, sample procurement, data collection, monitoring, data processing, toplines, <br /> crosstabs, and a survey findings download call and written report(memo or Powerpoint deck). <br /> Scope of work includes Service Provider research staff time devoted to the preparation, analysis, <br /> and delivery of such services. <br /> All Work will be completed by March 31, 2020, unless a written extension is granted by the <br /> City. <br />
