All applications must be submitted in hard copy at the permit counter. You will need to print out your permit application and submit it along with two copies of your site plan in order to make a complete application. Your driveway exhibit looks feasible to permit, however you will need to call out the proposed material of the driveway and the apron. The driveway apron must be concrete and it must meet City Standards. The particular driveway apron that you plan on installing must be called out on the site plan. Please see our 300 series (#315, 316, 317) to determine which apron you will be installing (most likely#316 due to grade at back of sidewalk). Please note that these additional site plan notes do not need to be drafted and can be added in by pen after printed. Once you have added these required elements to the site plan, please submit at the permit counter. We should be able to process your permit over the counter in about 20 minutes.
