Section 3.0 Statement of Compliance South Everett CADMAN Plant 16300 Glenwood Ave. I Everett, WA 98203 In accordance with W.AC. 296-62-07721 and PSCAA Regulation III, Article 4, Pacific Rim Environmental, Inc. performed an asbestos survey of the subject structures located at 6300 Glenwood Avenue in Everett, Washington 98203. Should employees or contract personnel encounter any suspect asbestos-containing materials(ACM) it their responsibility to: 1. Contact a representative of the owner. 2. Consult the inspection report to determine whether or not the suspect material contains asbestos. 3. If the suspect material does not appear in the inspection report, then that material was not sampled and must be presumed to contain asbestos until proven otherwise by sampling and PLM analysis. 4. Ensure that all employees and contractors are informed and advised of the location and type of materials that contain asbestos. Limitations: Sampling was performed in the following buildings only: Batch Tower, Pre-Stress Building and QC Lab, Batch Mixing Plant,Former Redi-Mix Warehouse, Former Color Mix Building The following Asbestos-containing materials were identified in the Batch Tower only: • Window Caulking(2nd Floor east end and center area south wall) • Popcorn Ceiling Texture(1g and 2"d Floor) • Texture(2"d Floor under cove base mastic) • Textured Coatings(Perimeter Wall,2"d floor west area) • Expansion Joint(2nd floor west wall between wail and column) • Cement Asbestos Board(Exterior 2"d floor west wail) • HVAC Sealant(Roof) • Texture on GWB(Break area east wall) The following Asbestos-containing materials were identified in the Pre-Stress Bldg.only; • Vent Sealant(Roof) The following Asbestos-containing materials were identified in the Batch Mixing Plant only: • Pipe Gasket(Boiler Room) I Hereby Attest The inspection report has been made available to me.
