EVERETT <br /> Elm WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2020 through December 31, 2020 <br /> Exhibit A <br /> Program Summary <br /> Organization& Program: Catholic Community Services—Clare's Place <br /> Program will provide housing support services to that will support an individual to reside indoors. The Residential <br /> Counselors primary responsibility is for daily crisis management,milieu relationship management, and effective and safe <br /> daily building operation at Clare's Place;a 65 unit,24/7 controlled access, permanent supportive housing project for highly <br /> vulnerable, chronically homeless adults,youth and veterans. <br /> The expected roles and activities of the Residential Counselor will include but are not limited to: <br /> • Interact and model appropriate professional behaviors with residents in the general milieu of Clare's <br /> Place common areas. <br /> • Coordinate and facilitate outside services with residents: <br /> o Contact outside service providers to schedule, confirm and manage a variety of appointments. <br /> • Provide outreach and engagement of service-resistant residents through creative, resourceful strategies <br /> that build trust and confidence. <br /> • Initiate, facilitate and promote on-site recreational and therapeutic activities (such as, but not limited to <br /> Community Garden &CD Group coordination and Social engagement& enrichment activities). <br /> • Ensure all functions in lobby office are run efficiently and effectively, including checking visitors in and <br /> out, answering telephones and monitoring security systems. <br /> • Acts as first responder to neighbor/community concerns promptly and professionally; ensure <br /> coordination with Supervisor and Housing Director as needed. <br /> • Maintain safety and security by monitoring all general access areas and enforcing building rules <br /> including street front. <br /> • Respond to emergencies and initiate action as required, including contact with emergency response <br /> systems; may include resident welfare checks. <br /> • Ensure significant events involving residents and building operation activities are recorded accurately in <br /> daily log: <br /> o Read daily logs upon shift entry <br /> o Pass on all necessary information during shift overlap for seamless coverage <br /> o Elevate issues as appropriate to Supervisor <br /> • Respond to resident complaints involving other residents; coordinate with Supervisor, CHS Building <br /> Manager, and/or community partners as needed. <br /> • Assess resident requests for maintenance, lease issues, rent payments, etc. in order to evaluate for <br /> appropriate assistance and/or direction to CHS building staff or other relevant services. <br /> • Participate in weekly staff meetings (including internal CCS programs, CHS staff as needed, and <br /> community partners). <br /> • Provide transportation to residents as needed/requested. <br /> CCS shall make every effort to engage participants in services to promote housing stability and prevent returns <br /> to homelessness. Support services shall emphasize participant choice and participation shall be entirely <br /> voluntary. <br /> 7 <br />
