State of Washington Department of Ecology <br />Agreement No: TCPRA-1921-EverPW-00080 <br />Project Title; <br />Recipient Name; <br />Task Number: 6 <br />Everett Landfill Tire Fire <br />CITY OF EVERETT <br />Task Title: CLEANUP CONSTRUCTION J006 <br />Task Cost: $325,000.00 <br />Page 14 of 27 <br />Task Description: <br />This task funds RECIPIENT's eligible costs ECOLOGY deems reasonable and necessary to complete the physical <br />construction of each cleanup action component required under the cleanup agreement for the Site, (CD No. 01 2 03640 6), <br />including any compliance monitoring during physical construction. Eligible costs may include: <br />• Permit applications for constructing a cleanup action component. <br />• Physical construction of a cleanup action component, such as removing and disposing of contaminated material or installing a <br />treatment system, engineered control, or monitoring network. <br />Far example, for monitored natural attenuation or recovery, eligible costs may include installing an initial network of monitoring <br />wells to evaluate the progress of attenuation or recovery. <br />• Physical construction of a stormwater source reduction and treatment facility required as part of the cleanup action, such as <br />an absorption unit or stormwater pond. <br />• Protection monitoring to confirm that human health and the environment are adequately protected during physical construction <br />of a cleanup action component as specified in the health and safety plan. This includes any sampling and analysis or other <br />testing. <br />• Performance monitoring during physical construction to determine whether a cleanup action component has attained or is <br />attaining cleanup standards and, if appropriate, remediation levels or other performance standards. This includes any sampling <br />and analysis or other testing. <br />• Establishment of an institutional control, including developing and filing an environmental covenant or alternative mechanism. <br />• Construction oversight, documentation, surveying and mapping, and quality assurance and control. <br />• Project management and coordination with ECOLOGY during construction. <br />Task Goal Statement; <br />To complete the cleanup actions in accordance with the cleanup action plan, the engineering design specifications, construction <br />plans, and standard safety and engineering practices. <br />Task Expected Outcome: <br />The completed cleanup will be protective of human health and the environment and meet Madel Toxics Control Act cleanup <br />levels. <br />Recipient Task Coordinator; Randy Loveless, Hanna Lintukorpi <br />CLEANUP CONSTRUCTION - J006 <br />Deliverables <br />Nunnber Description Due Date <br />6A Construction Completion Report <br />Template Version 10l30l2015 <br />
