Fisk, <br /> (425) 257—8909 or gfisk@everettwa.gov. <br /> 2.0 Bidder's Check List <br /> The bidder's attention is directed to the following City-provided forms which must be executed in full as <br /> required and submitted with the bid: <br /> 1. Proposal: The lump sum and unit price items must be shown in the space provided. Show unit prices <br /> in figures. <br /> 2. Proposal Signature Sheet: To be filled in and signed by the Bidder. <br /> 3. Subcontractor List: To be filled in by the Bidder. <br /> 4. Local Agency Disadvantaged Business Enterprise Utilization Certification <br /> (DOT Form 272-056): To be filled in and signed by the bidder(if DBE goal is assigned.) <br /> 5. Local Agency Disadvantaged Business Enterprise (DBE) Written Confirmation Document (DOT Form <br /> 422-031): To be filled in and signed by DBEs (if DBE goal is assigned.) <br /> 6. Non-Collusion Declaration: To be submitted with the bid. <br /> 7. Bid Bond: This form is to be executed by the Bidder and the surety company unless bid is accompanied <br /> by a certified check. The amount of this bond shall be not less than five percent (5%) of the total <br /> amount bid and may be shown in dollars or on a percentage basis. Cash will not be accepted. <br /> Failure to complete the aforementioned forms and to submit said forms with the bid may be due cause for <br /> rejection of bid. <br /> 3.0 Pre-Award Forms <br /> The following form is required to be signed and submitted prior to award of Contract: <br /> 1. Certification of Compliance with Wage Payment Statutes: To be filled in and signed. This certification <br /> is not required to be submitted with the bid proposal and may be submitted after bid opening. The <br /> Contract cannot be awarded without this certification. <br /> 4.0 Contract Forms <br /> The following forms are to be executed after the award of Contract: <br /> 1.
