1 trucks owned and leased by the DBE must be exclusively employed by 2 the DBE and reflected on the DBE's payroll. 3 i 4 • Lease agreements for trucks shall indicate that the DBE has exclusive 5 use of and control over the truck(s). This does not preclude the leased 6 truck from working for others provided it is with the consent of the DBE I 7 and the lease provides the DBE absolute priority for use of the leased 8 truck. 9 I 10 • Leased trucks shall display the name and identification number of the 11 DBE. 12 13 UDBE/DBE/FSBE Truck Unit Listing Log 14 In addition to the subcontracting requirements of Section 1-08.1, each DBE 15 trucking firm shall submit supplemental information consisting of a completed 16 Primary UDBE/DBE/FSBE Truck Unit Listing Log (WSDOT Form 350-077), copy I 17 of vehicle registrations, and all Rental/Lease agreements (if applicable). The 18 supplemental information shall be submitted to the Engineer prior to any trucking 19 services being performed for DBE credit. Incomplete or incorrect supplemental I 20 information will be returned for correction. The corrected Primary 21 UDBE/DBE/FSBE Truck Unit Listing Log and any Updated Primary 22 UDBE/DBE/FSBE Truck Unit Listing Logs shall be submitted and accepted by I 23 the Engineer no later than ten calendar days of utilizing applicable trucks. 24 Failure to submit or update the DBE Truck Unit Listing Log may result in trucks 25 not being credited as DBE participation. 26 27 Each DBE trucking firm shall complete a Daily UDBE/DBE/FSBE Trucking Unit 28 Listing Log for each day that the DBE performs trucking services for DBE credit. 29 The Daily UDBE/DBE/FSBE Trucking Unit Listing Log forms shall be submitted I 30 to the Engineer by Friday of the week after the work was performed. 31 32 Joint Checking I 33 A joint check is a check between a Subcontractor and the Contractor to the 34 supplier of materials/supplies. The check is issued by the Contractor as payer 35 to the Subcontractor and the material supplier jointly for items to be incorporated I 36 into the project. The DBE must release the check to the supplier, while the 37 Contractor acts solely as the guarantor. 38 I 39 A joint check agreement must be approved by the Engineer and requested by 40 the DBE involved using the DBE Joint Check Request Form (form # 272-053) 41 prior to its use. The form must accompany the DBE Joint Check Agreement 1 42 between the parties involved, including the conditions of the arrangement and 43 expected use of the joint checks. 44 45 The approval to use joint checks and the use will be closely monitored by the I 46 Engineer. To receive DBE credit for performing a CUF with respect to obtaining 47 materials and supplies, a DBE must "be responsible for negotiating price, 48 determining quality and quantity, ordering the material, installing and paying for I 49 the material itself."
