Exhibit "A" <br /> Scope of Services,Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Rental Assistance program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Rental Assistance will serve 32 Household by providing rental assistance. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Hand in Hand's method is to build relationships with at-risk families living in poverty and affected by the <br /> COVID-19 Global Pandemic through on and offsite direct activities, referrals from area schools, social <br /> service agencies, and the Department of Children, Youth and Families. Using a case management <br /> approach, we will evaluate the family need, provide direct services & resources and/or refer to partner <br /> agencies for additional support. Hand in Hand will monitor CDBG funds and review quarterly (at a <br /> minimum) to ensure all funds are disbursed during the reporting period. The emergency basic needs of <br /> our clients will outweigh the available funding. <br /> B. National Objectives <br /> The Subrecipient certifies that the activities carried out with funds provided under this Agreement will <br /> meet the following CDBG National Objective, as identified by the City of Everett for priority in the 2019 <br /> Program Year, "Benefit low/moderate income persons" - as defined in 24 CFR Part 570.208. <br /> Appropriate Subcategory and Matrix Code under the National Objective: Limited Clientele <br /> 05Q: Subsistence Payments 24 CFR 570.207(b)(4) or 42 USC 5305(a)(8) + 24 CFR 570.482(c)(2) <br /> - One-time or short-term (no more than three months) emergency payments on behalf of <br /> individuals or families, generally for the purpose of preventing homelessness. <br /> Description of how the project will meet this subcategory: Our outreach team will work with Everett <br /> families in crisis due to the effects of the COVID-19 Global Pandemic focusing on housing and <br /> food security issues. Through our case management process which includes bi-weekly check- <br /> ins, review of circumstances and relationship building we will determine ongoing ability to <br />
