Form 1024(Rev.1-2018) Page 2 <br /> Part II.Activities and Operational Information (Must be completed by all applicants) <br /> 1 Provide a detailed narrative description of all the activities of the organization—past,present,and planned.Do not merely refer to or repeat the <br /> language in the organizational document.List each activity separately in the order of importance based on the relative time and other resources <br /> devoted to the activity.Indicate the percentage of time for each activity.Each description should include,as a minimum,the following:(a)a <br /> detailed description of the activity including its purpose and how each activity furthers your exempt purpose;(b)when the activity was or will be <br /> initiated;and(c)where and by whom the activity will be conducted. <br /> The sole purpose and activity of the City of Everett Fire VEBA Trust("Trust")is to fund the benefits provided through four benefit plans(the <br /> "Plans")sponsored by the City of Everett.The Plans are Health Reimbursement Arrangements pursuant to Revenue Ruling 2002-41 and IRS <br /> Notice 2002-45. They provide to certain employees and former employees of the City of Everett benefits consisting of reimbursement of <br /> certain health care expenses.Contributions to the Plans by the City of Everett are placed in the Trust for the purpose of paying benefits <br /> under the Plans when due.The assets of the Trust may be invested with the intent that any earnings will accrue on a tax-free basis under <br /> section 501(c)(9)of the Internal Revenue Code. <br /> The City of Everett is the plan administrator of the Plans. <br /> Benefit Plans Administrative Services,Inc.acts as a third-party claims administrator under the Plans. <br /> The trustee of the Trust is Hand Benefits&Trust Company. <br /> The Trust was initially funded by the City of Everett on 01/10/2019.
