Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Hotel Vouchers program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Hotel Vouchers will serve 60 individuals/households by providing emergency motel vouchers to <br /> homeless individuals and those experiencing housing challenges in the City of Everett. <br /> General Administration <br /> The Emergency Motel Voucher Program at Compass Health is administered by the Housing <br /> Program staff who receive referrals for vouchers from Everett agencies and community partners for <br /> Medicaid/low income, homeless individuals/families, or for those with housing challenges. Compass <br /> Health housing staff screen each individual/family for motel placement based on available motel <br /> capacity and ability to abide by the rules of the motel. Each voucher provides an individual/family a <br /> motel room for one week. <br /> B. National Objectives <br /> The Subrecipient certifies that the activities carried out with funds provided under this Agreement will <br /> meet the following CDBG National Objective, as identified by the City of Everett for priority in the <br /> 2019 Program Year, "Benefit low/moderate income persons" - as defined in Emergency. <br /> Appropriate Subcategory and Matrix Code under the National Objective: Limited Clientele <br /> 05Z: Other Public Services Not Listed in 03T and 05A-05Y 24 CFR 570.201(e) or 42 USC <br /> 5305(a)(8) + 24 CFR 570.482(c)(2) <br /> Description of how the project will meet this subcategory: All services at Compass Health <br /> support low income and/or Medicaid recipients. The Compass Health Emergency Motel <br /> Voucher Program provides emergency motel vouchers to homeless individuals and those <br /> experiencing housing challenges. Because of COVID-19, there is an increased need for <br /> emergency motel vouchers in the City of Everett. <br />
