F1��- <br /> W TCH <br /> Helping the Helpers <br /> Regional Influenza Network (RIN) Participation Form <br /> The FirstWatch Regional Influenza Network (or simply, RIN) uses FirstWatch Real-Time <br /> surveillance software to automatically monitor data in 911, EMS, Fire, and Police data systems, <br /> watching for increases in complaints or symptoms that are consistent with the flu, whether they <br /> appear in geographic clusters or across the entire population being monitored. <br /> As the Regional Influenza Network continues to provide real-time analysis of influenza-like- <br /> illness symptoms from 911 data systems, the network is not only growing but also evolving. By <br /> agreeing to share certain information for the purpose of participating in the Regional Influenza <br /> Network, you will in turn have access to matching information from other organizations that have <br /> elected to share their data on a regional or national basis. <br /> As a RIN member, you will be granted access to one or more standard RIN triggers that are <br /> based on the latest CDC and WHO definitions for influenza like illness, and may be refined over <br /> time. These "base" triggers may vary slightly depending on the data source(s) FirstWatch <br /> actively monitors for each agency. Each participating agency will also have the opportunity to <br /> access certain deidentified triggers and aggregated views for other participating agencies and <br /> regions, as well as the ability to set alerting thresholds for their local RIN triggers, and subscribe <br /> to alerts from other RIN triggers with standard alerting thresholds. FirstWatch's approach to de- <br /> identifying data is to present all the information from the records, with the exception of street <br /> addresses, patient telephone numbers, patient names and other information such as DOB, <br /> Social Security/Insurance Numbers, latitude/longitude and free-text comment or narrative fields. <br /> In addition to having access to triggers with alerting functionality, all RIN members will have the <br /> ability to display deidentified data on a heat map or cluster view, showing all calls that meet the <br /> criteria for influenza like illness. <br /> As always, FirstWatch commits to providing opportunities to meet and collaborate with other <br /> participating agencies around the US and Canada to gain additional insight and perspective as <br /> we all prepare for each year's flu season. <br /> By signing this participation form, I agree to the terms and conditions listed above. <br /> Agency: C I f y • v' 2-E4-G f <br /> By: ).. <br /> Office of the City Attorney <br /> Name: C ! j•G 1 '�T�r L c %V APPROVED AS TO FORM <br /> David C.
