PROCUREMENT <br /> EvERETT EMERGENCY PURCHASE RATIFICATION <br /> WASHINGTON <br /> An emergency purchase is defined as: <br /> A purchase in response to unforeseen circumstances,beyond the control of the City of Everett which <br /> A. presents a real,immediate threat to the proper performance of essential functions or <br /> B. will likely result in material loss or damage to property,bodily injury,or loss of life if immediate action is not <br /> taken. <br /> All emergency purchases shall follow the City of Everett Standard Procurement Policy&Federal Emergency Contracting <br /> Policy Section 9.6. <br /> I REQUEST THAT THE REFERENCED APPROVED EMERGENCY PURCHASE BE RATIFIED. <br /> Emergency Approval Date: 1-4-2021 <br /> Location of Emergency:SE Everett Mall Way—4th Ave W <br /> Supplier Name:Service Electric Co,Inc <br /> State the facts that constituted the Traffic signal cabinet and foundation were knocked down due to <br /> emergency vehicle crash.Traffic signal had no power and could not operate until <br /> . <br /> a new foundation was installed and a new traffic signal cabinet was <br /> Attach a copy of the Emergency installed.This presented a real,immediate threat to the proper <br /> Approval justification form. performance of the essential function of the traffic signal that was <br /> currently not functioning and would result in damage of property, or <br /> bodily injury if immediate action had not been taken. Leaving a 7-lane <br /> intersection without traffic signal control increases the opportunity <br /> for additional pedestrian and vehicular collisions.This increases the <br /> City's liability as well as the potential for bodily injury and damage to <br /> property.This work needed to be done immediately to minimize <br /> impact. <br /> What was the final cost of the $21,884.24 <br /> emergency? <br /> STATEMENT OF NEED AND CERTIFICATION: <br /> My department requests that the waiver of competitive bid requirements be ratified. I know of no conflict of interest on <br /> my part or personal involvement in any way with this request. No gratuities,favors or compromising action have taken <br /> place.
