Em EVERETT <br /> MEM WASHINGTON <br /> City of Everett Human Needs Grant <br /> January 1, 2021 through December 31, 2021 <br /> Exhibit A <br /> Program Summary <br /> Organization&Program: Cocoon House:Cocoon House Shelter <br /> Cocoon House provides emergency housing for Everett youth who are experiencing homelessness due to abuse and <br /> neglect,severe family conflict,family homelessness,and other contributing factors. Under this funding, housing will be <br /> provided through the agency's Cocoon House Central shelter and in Host Homes within the community.The program is <br /> designed to provide up to 21 nights of housing.Youth have immediate,24-hour access and transportation to emergency <br /> housing through the agency's Safe Place line. <br /> The program provides more than just a safe place to sleep:youth receive individualized case management,education and <br /> employment support, behavioral and physical health care and referrals,life skills training,and basic needs items. Family <br /> reunification services are provided whenever there is a possibility of a youth returning to their family home.This funding <br /> will be used to cover staffing costs. <br /> 7 <br />
