Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Project Description <br /> The Subrecipient will be responsible for administering the Independent Living program in a manner <br /> satisfactory to the City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the Community Development <br /> Block Grant Program: <br /> Program Delivery <br /> Activity: <br /> The Independent Living will serve 125 people and provide 175 units of service by providing systems <br /> navigation to many confusing and often cumbersome systems that many families and individuals <br /> with disabilities don't understand. Guardianship, SSI, DDA (Developmental Disabilites <br /> Administration), Special needs trusts, and DVR. By providing this critical information to families <br /> raising an someone with a disability it is key to living an independent life and understanding the <br /> many systems necessary to achieve this independence. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Appointments, phone conversation and workshops (currently happening via Zoom) <br />
