I CITY OF EVERETT SPECIAL PROVISIONS The Proposal Form identifies the project and its location and describes the work. It also lists estimated quantities, units of measurement, the items of work, and the materials to be furnished at the unit bid prices. The Bidder shall complete spaces on the proposal form that call for, and are not limited to, unit prices; extensions; summations; the total 1 bid amount; signatures; date; and, where applicable, retail sales taxes and acknowledgment of addenda; the bidder's name, address, telephone number, and signature; the bidder's UDBE/DBE/M/WBE commitment, if applicable; a State of Washington Contractor's Registration Number; and a Business License Number, if applicable. Bids shall be completed by typing or shall be printed in ink by hand, preferably in black ink. The required certifications are included as part of the Proposal Form. Bidder shall submit Bidder's Proposal on the Proposal Form provided in the Contract Documents. 1-02.6 Preparation of Proposal Delete "unless it approves in writing" from the second sentence of the first paragraph of 1- 02.6. Revise the fourth paragraph of 1-02.6, beginning with "The Bidder shall submit with the Bid a completed Underutilized Disadvantaged Business Enterprise (UDBE) Utilization Certification", to read as follows: Contractor agrees that the Contractor shall actively solicit the employment of minority group members. Contractor further agrees that the Contractor shall actively solicit Bids for the subcontracting of goods or services from qualified minority businesses. Contractor shall furnish evidence of the Contractor's compliance with these requirements of minority employment and solicitation. Contractor further agrees to consider the grant of subcontracts to said minority bidders on the basis of substantially equal proposals in the light most favorable to said minority businesses. The Contractor shall be required to submit evidence of compliance with this section as part of the Bid. As used in this section, the term "minority business" means a business at least 51 percent of which is owned by minority group members. Minority group members include, but are not limited to, African-Americans, women, native Americans, Asians, Eskimos, Aleuts, and Hispanics. Delete the sixth paragraph of 1-02.6, which begins with "The Bidder shall submit with their Bid a completed Contractor Certification Wage Law Compliance form (WSDOT Form 272-009)." Revise the language that states: "The Bidder shall submit with the Bid a list of: 1. Subcontractors who will perform the work of heating, ventilation and air conditioning, plumbing as described in RCW 18.106, and electrical as described in RCW 19.28, and" to read as follows "The Bidder shall submit with the Bid on the form provided by the City a list of: 1. Subcontractors who will perform the work of heating, ventilation and air conditioning, plumbing as described in RCW 18.106, electrical as described in RCW 19.28, structural steel installation, and rebar installation, and" Water Main Replacement"V" Division 1 —GENERAL REQUIREMENTS April,2021 WO No—UP3730 SP—43
