Exhibit "A" <br /> Scope of Services, Time of Performance, Budget, <br /> Payment, Notices, and Representatives <br /> I. Scope of Service <br /> A. Activities/Protect Description <br /> The Subrecipient will be responsible for administering the WayOut program in a manner satisfactory <br /> to the City and consistent with any standards required as a condition of providing these funds. Such <br /> program will include the following activities eligible under the Community Development Block Grant <br /> Program: <br /> Program Delivery <br /> Activity: <br /> The WayOut will serve one hundred (100) Household by providing up to ten (10)WayOUT seminars <br /> for low-risk youth offenders/at-risk youth and their parents/caregivers. Services will be delivered with <br /> the goals of reducing recidivism, reducing/avoiding youth homelessness, and improving supportive <br /> relationships within families. <br /> General Administration <br /> [Description of general administrative services to be performed in support of activities noted above] <br /> Cocoon House provides adequate administrative support for the operation of the WayOUT program <br /> including leadership, human resources, and sound financial practices. Cocoon House maintains <br /> agency policies and procedures which include an affirmative action plan, a non-discrimination policy, <br /> and grievance procedure. The agency also maintains all necessary licenses and certifications <br /> including city and state business licenses. Program staff receive regular supervision and professional <br /> development opportunities. Program activities are overseen by agency leadership and frequent <br /> progress reports are provided to the Board of Directors. <br /> Cocoon House maintains an accounting, financial reporting, and financial management and <br /> operations system in order to comply with Generally Accepted Accounting Principles (GAAP). <br /> Cocoon House's governing body, officers, and management personnel exercise due vigilance and <br /> diligence to assure that all financial transactions, accounts, records, and reports fully comply with <br /> these standards. All uses of program income are accurately reflected in the agency's records and <br /> are subject to review and audit by an independent auditor and agents of granting authority. Cocoon <br /> House secures a comprehensive, independent audit of its operations and programs annually. <br /> B. National Objectives <br />
