WAShO98034 FNt: 120EI E23-2203 EVERETT; 120EI2E9-0817 <br />October 18, 1996 <br />Cert No. 9610-31E <br />Far West Industries <br />2611 Pacific Avenue <br />Tacoma, Washington 98402 <br />Reference: Load Testing -Seismic Wall Anchors <br />McCabes Project - Everett <br />Gentlemen; <br />In accordance with your request and as outlined by the engineers representative load testing <br />was performed on seismic wall anchor bolts as located on the east and west walls of the above <br />noted structure. <br />The anchor bolts were adhered into the brick masonry by a cementitous material. <br />The pull tests were performed as outlined on plans S-1 Structural notes 1.3.2 and 1.4.
