Environmental Engineering <br />Construction Testing and Inspection <br />Field <br />Report NO.: 09618358DFR7242018EL <br />DATE: 7/24/2018 Set Count: CONTRACTOR: Clete M Construction <br />PROJECT #: 09618358 PERMIT #: C18 02-003/004/005 <br />PROJECT: Colby Ave Residence INSPECTOR: Edan Lewis <br />LOCATION: 3710-3714 Colby Ave JURISDICTION: Everett <br />KA PAL JM WEATHER: Sunny TEMP: 76' F <br />Krazan representative arrived on site as requested for proprietary anchor inspection for the new foundation footing that will <br />support a future shear wall located in the basement of the building. Upon arrival, contractor had drilled 56+ holes into the <br />existing slab for the placement of #4 reinforcing steel for the connection into existing slab in preparation of future new <br />footings to be installed. Newly installed anchors had holes drilled with minimum of 4" embedment and #4-18" long rebar <br />was installed into the existing slab. Simpson Strong -tie Set-XP epoxy grout was used for all the anchors installed today. <br />Representative observed the cleaning out of the holes, the embedment depth and the placement of the epoxy grout and <br />rebar. Anchors were installed per manufacturer and industry standards. <br />Representative also arrived on site as scheduled for reinforcing steel placement. Reinforcing steel inspected today covered <br />foundation footings located in the locations referenced above. Also inspected HTTX bolt tie -downs that were installed per <br />spec.
