FORM 4.03 PRICE SHEET REQUEST FOR PROPOSAL #2021-002 AUTOMATED PARKING MANAGEMENT SYSTEM Supplier Name: OSP US, Inc. d/b/a Orbility Prices must include providing all services as detailed in the Scope of Work. 1. Complete the price sheet. 2. Provide a firm fixed, not to exceed, lump sum amount for each deliverable in Section A. Prices must include all requirements listed in Section 2 unless noted. The three deliverable prices must add up to the not to exceed amount. Include 2-year parts and labor warranty for both site and shop labor in the equipment delivery pricing. 3. Clearly identify any services mentioned in your response that are not included in your proposed fee such as services that would be an additional expense. A. Additional Service Task Breakdown Cost Deliverable: Installation Manager Trip - Confirm Site Equipment Locations with Contractor (prior to civil work) $ 1,800.00 Deliverable: Installation Manager Trip - Confirm Site Readiness Walk Through (two weeks prior to installation) $ 2,500.00 Deliverable: Training — include options and prices for the options Option 1: Onsite Instructor led training with 6 classes of 2 hours each across 2 contiguous days $ 6,765.00 Option 2: On Line Training with 6 classes of 2 hours each across 2 days $ 2,640.00 Deliverable : Annual Maintenance $ 20,401.91 Describe and provide pricing for a maintenance program. Include what will be covered, the days and hours service calls will be covered and promised response time. Subtotal: $235,010. 71 .tsio i ax: $20,951.95 Tota : $255,962.66 Any service request will be responded to within two (2) business hours by our representative located at 606 120th Avenue NE #D-201, Bellevue, Washington 98005. For software issues, remote support will commence within four (4) business hours. For hardware issues, onsite technicians will be onsite within one (1) business day to commence work. Time to resolution for onsite work for a standard part is 24 hours. Non-standard parts or severe abuse/vandalism may take up to 7 business days. All service work is to be performed between the hours of 8:00 am and 5:00 pm, Monday through Friday (business hours). Covered services are included in the annual fee. Non -covered services are performed at an hourly rate of $125 per hour. Evening hours between 5:00 pm and 11:00 pm will be performed at $125 per hour for covered services and $225 per hour for non -covered services. Any weekend work or work done between the hours of 11:00 pm and 8:00 am will be performed at $225 per hour for covered services and $300 per hour for non -covered services. Any issue not necessitated because of abuse or neglect are considered Covered Services and will be taken care of through the Annual Maintenance Agreement. The replacement of ticket or receipt paper is not included in Covered Services. Any service performed outside of regular business hours has a 2-hour minimum. Non -covered service performed during regular business hours has a half-hour minimum. After the first year of annual maintenance (which occurs after the initial two year warranty) the annual maintenance price will escalate by 5% per year. ADDITIONAL SERVICE NOT INCLUDED ABOVE: For the processing of credit card payments, the City of Everett will have a direct agreement with TNS, a payment gateway. This agreement will include a monthly fee and a per transaction fee based on the quantity of credit card terminals installed. For example for an installed base of 5 - 10 devices, there will be a $15.00 monthly recurring fee and a $0.08 per transaction fee.
