Application for an On -site Sewage System Permit <br />Supplemental Information Report <br />A Health District sewage disposal installation permit: <br />a. Will not be issued before the city/county building permit is issued; <br />b. Will remain valid concurrent with the city/county building permit; <br />c. Will not be issued before the applicable Health District installation permit is paid. This <br />permit fee is required for all systems except repairs and alterations. <br />d. Will not be issued until a conformed copy of the recorded monitoring and maintenance <br />Declaration of Covenant (Copy Enclosed) is submitted to the Snohomish Health District. <br />This covenant is required for all systems except gravity and pressure distribution. <br />e. Will not be issued until a copy of a signed monitoring and maintenance service <br />agreement with an approved service provider is submitted to the Snohomish Health <br />District. This agreement is required for all systems except gravity and pressure <br />distribution. <br />If the building permit is withdrawn, revoked, or expires, the installation oe�will no <br />longer be valid. <br />• On -site sewage disposal systems shall be installed by installers certified by the Health District. <br />A current list of certified installers is available at www.snohd.ora (under Septic select Septic <br />Contactors. <br />• A request for the inspection of an on -site sewage disposal system installation must be made to <br />the system designer or engineer, who will, upon their inspection and approval of the installation, <br />request final inspection and approval from the Health District.
