ATTACHMENT H COORDINATED COMMUNITY RESPONSE TEAM PARTICIPATION (All Applicants) Only one completed form needs to be submitted for your CCR team. Decide which agency will respond on behalf of all STOP Grant applicants in your county. The designated agency must submit this completed CCR form with their application. A copy of the submitted form should be provided to the other STOP Grant applicants in your county for their files. Please attach additional pages if more space is needed. Agency to submit completed form in their application: City of Everett County: Snohomish Date Completed: 10/6/21 A condition of receiving these STOP Formula Grant funds is mandatory participation in the CCR meetings and meaningful involvement as a member of the county or tribal CCR team,which must meet at least quarterly during the grant period. While it is a federal requirement that recipients report on their progress in reducing, or reducing the risk of, domestic violence related homicides, CCR teams should also consider and address services for youth or adult victims of domestic violence,sexual assault, dating violence, or stalking. Some goal examples could include: • Map system responses to identify barriers, gaps, and intervention points • Reviewing and revising policies, practices, and protocols • Conduct coordinated case reviews and case management • Share, review and discuss data (consider: What does it reveal?Where are the gaps? What will be done?) • Implement risk or lethality assessments throughout the community system—could include victim services, police calls for service, prosecution referrals • Review and implement promising practices • Conduct high-risk offender reviews and identify how to maximize their victims safety Goals should be specific, measurable, attainable, relevant, and time-based.
