ATTACHMENT H <br /> COORDINATED COMMUNITY RESPONSE TEAM PARTICIPATION <br /> (All Applicants) <br /> Only one completed form needs to be submitted for your CCR team. Decide which agency will respond on behalf <br /> of all STOP Grant applicants in your county. The designated agency must submit this completed CCR form with <br /> their application. A copy of the submitted form should be provided to the other STOP Grant applicants in your <br /> county for their files. Please attach additional pages if more space is needed. <br /> Agency to submit completed form in their application: City of Everett <br /> County: Snohomish <br /> Date Completed: 10/6/21 <br /> A condition of receiving these STOP Formula Grant funds is mandatory participation in the CCR meetings and <br /> meaningful involvement as a member of the county or tribal CCR team,which must meet at least quarterly during <br /> the grant period. <br /> While it is a federal requirement that recipients report on their progress in reducing, or reducing the risk of, <br /> domestic violence related homicides, CCR teams should also consider and address services for youth or adult <br /> victims of domestic violence,sexual assault, dating violence, or stalking. Some goal examples could include: <br /> • Map system responses to identify barriers, gaps, and intervention points <br /> • Reviewing and revising policies, practices, and protocols <br /> • Conduct coordinated case reviews and case management <br /> • Share, review and discuss data (consider: What does it reveal?Where are the gaps? What will be done?) <br /> • Implement risk or lethality assessments throughout the community system—could include victim <br /> services, police calls for service, prosecution referrals <br /> • Review and implement promising practices <br /> • Conduct high-risk offender reviews and identify how to maximize their victims safety <br /> Goals should be specific, measurable, attainable, relevant, and time-based. <br /> During the grant period, CCR teams will be required to report on their activities.
