With the addiuon of the following, the plans are approved. <br />1. The Health District operadng perntit applicadon process must be completed prior to opening for business. <br />2. An indirect waste is required for the food preparation sink, mechanical dishwasher, ice machine, running <br />water dipper well, and equipment in which food is placed. <br />3. A reduced pressure backflow prevention device is required at the end of the copper water pipe serving the pop <br />dispensing system prior to [he carbonation device. <br />4. A horizontal separation of at least l6 inches or a vertical partition 16 inches in height is required between the <br />food preparation sink and all other sinks. <br />5. Food preparation tables must be NSF listed stainless steel tables. <br />6. The ceiling above the food preparation areas must be nonabsorbeut, smooth, and easily cleanable. <br />7. A pre-ri.�s, sink wi[h a spray atm is required in line with the kitchen dish machine. <br />A preoperaticnal inspection is required prior tu openiug for business. At the time of inspection the <br />construction of the food service establishment must be complete aud all equipment must be in place. Incomplete <br />conswction may result in a S130.(� reinspection fee. Contact the Food Program office a miaimum of one <br />week in advance to schedule an appointment. This will ensure compliance with the Rules �nd Rep,�lations of <br />rhr St�te Bo1rd of Health for Food �ervice Sanitation. <br />If there are any changes or additions ro the approved layout or equipment, the Snohomish Health Disvict must be <br />notified. <br />Please contact me if you have any questions.
