EXISTING ACTIVE AND ABANDONED UTILITY LOCATIONS SHOWN ON PLANS ARE COMPILED FROM SITE SURVEY AND <br />AVAILABLE AS -BUILT INFORMATION. THE CONTRACTOR SHALL DETERMINE THE EXACT LOCATION OF ALL EXISTING <br />ACTIVE AND ABANDONED UTILITIES BEFORE COMMENCING WORK AND SHALL BE FULLY RESPONSIBLE FOR ANY AND <br />ALL DAMAGES THAT HAPPEN DUE TO THE CONTRACTOR'S FAILURE TO LOCATE EXACTLY AND PRESERVE ANY AND <br />ALL UNDERGROUND UTILITIES. <br />EARTHWORK NOTES <br />1. DO NOT COMMENCE EARTH MOVING OPERATIONS UNTIL TEMPORARY EROSION AND SEDIMENTATION CONTROL (ESC) <br />MEASURES ARE IN PLACE. <br />2. PROTECT AND MAINTAIN EROSION AND SEDIMENTATION CONTROLS DURING EARTH MOVING OPERATIONS. <br />3. SUBGRADE PREPARATION, PLANTING MATERIAL DEPTH AND SECTION AND SHALL BE IN ACCORDANCE WITH <br />LANDSCAPE PLANS. <br />4. EXCAVATION AND TRENCHING DURING WET CONDITIONS SHALL COMPLY WITH THE CONSTRUCTION GENERAL <br />STORMWATER PERMIT AND CITY OF EVERETT PUBLIC WORKS PERMIT. <br />TEMPORARY EROSION AND SEDIMENT CONTROL NOTES <br />1. APPROVAL OF THIS EROSION/SEDIMENTATION CONTROL (ESC) PLAN DOES NOT CONSTITUTE AN APPROVAL OF PERMANENT ROAD OR <br />DRAINAGE DESIGN (E.G. SIZE AND LOCATION OF ROADS, PIPES, RESTRICTORS, CHANNELS, RETENTION FACILITIES, UTILITIES). <br />2. THE IMPLEMENTATION OF THESE ESC PLANS AND THE CONSTRUCTION, MAINTENANCE, REPLACEMENT, AND UPGRADING OF THESE ESC <br />FACILITIES IS THE RESPONSIBILITY OF THE APPLICANT/CONTRACTOR UNTIL ALL CONSTRUCTION IS COMPLETED AND APPROVED AND <br />VEGETATION/LANDSCAPING IS ESTABLISHED. <br />3. THE BOUNDARIES OF THE CLEARING LIMITS SHOWN ON THIS PLAN SHALL BE CLEARLY FLAGGED IN THE FIELD PRIOR TO CONSTRUCTION. <br />DURING THE CONSTRUCTION PERIOD, NO DISTURBANCE BEYOND THE FLAGGED CLEARING LIMITS SHALL BE PERMITTED. THE FLAGGING <br />SHALL BE MAINTAINED BY THE APPLICANT/CONTRACTOR FOR THE DURATION OF CONSTRUCTION. <br />4. THE ESC FACILITIES SHOWN ON THIS PLAN MUST BE CONSTRUCTED IN CONJUNCTION WITH ALL CLEARING AND GRADING ACTIVITIES, AND IN <br />SUCH A MANNER AS TO ENSURE THAT SEDIMENT AND SEDIMENT LADEN WATER DO NOT ENTER THE DRAINAGE SYSTEM, ROADWAYS, OR <br />VIOLATE APPLICABLE WATER QUALITY STANDARDS. <br />S. THE ESC FACILITIES SHOWN ON THIS PLAN ARE THE MINIMUM REQUIREMENTS FOR ANTICIPATED SITE CONDITIONS. DURING THE <br />CONSTRUCTION PERIOD, THESE ESC FACILITIES SHALL BE UPGRADED AS NEEDED FOR UNEXPECTED STORM EVENTS AND TO ENSURE THAT <br />SEDIMENT AND SEDIMENT -LADEN WATER DO NOT LEAVE THE SITE. <br />6. THE ESC FACILITIES SHALL BE INSPECTED ROUTINELY AND MAINTAINED BY THE APPLICANT/CONTRACTOR TO ENSURE THEIR CONTINUED <br />FUNCTIONING, ESPECIALLY AFTER STORM EVENTS. <br />7. THE ESC FACILITIES ON INACTIVE SITES SHALL BE INSPECTED AND MAINTAINED WITHIN THE 48 HOURS FOLLOWING A RAIN EVENT AND AS <br />OTHERWISE NOTED IN THE SWPPP . <br />8. AT NO TIME SHALL MORE THAN ONE FOOT OF SEDIMENT BE ALLOWED TO ACCUMULATE WITHIN A TRAPPED CATCH BASIN. ALL CATCH <br />BASINS AND CONVEYANCE LINES SHALL BE CLEANED PRIOR TO PAVING. THE CLEANING OPERATION SHALL NOT FLUSH SEDIMENT LADEN <br />WATER INTO THE DOWNSTREAM SYSTEM. <br />9. A STABILIZED CONSTRUCTION ENTRANCE IS OMITTED FROM THIS PROJECT. APPROXIMATELY 700 LINEAL FEET OF STABILIZED INTERNAL <br />ROADS ARE AVAILABLE TO REMOVE SEDIMENT FROM TIRES. IN ADDITION, CONSTRUCTION EQUIPMENT SHALL ONLY MOVE TO AND FROM <br />THE PROJECT AREA ALONG THE STABILIZED ROAD AND SHALL NOT BE ALLOWED TO DRIVE ONTO THE EXISTING ASPHALT SURFACE WEST OF <br />THE PROJECT LIMITS. THE CONTRACTOR SHALL PLACE TEMPORARY CHANNELIZATION DEVICES AT THIS LOCATION TO PREVENT VEHICLES <br />FROM TRANSPORTING SEDIMENT TO THE AREA TRIBUTARY TO THE STORM SYSTEM. ADDITIONAL MEASURES MAY BE REQUIRED TO ENSURE <br />THAT ALL PAVED AREAS ARE KEPT CLEAN FOR THE DURATION OF THE PROJECT. <br />10. THE CONTRACTOR SHALL PROVIDE PERIODIC STREET CLEANING TO REMOVE DEBRIS AND SEDIMENT TRACKED OFF THE SITE. <br />11. APPROPRIATE MEASURES SHALL BE TAKEN TO STOP SEDIMENT FROM ENTERING SURFACE WATER BODIES IF THE PROPOSED BMPS FAIL. <br />12. BARE AND/OR DISTURBED SOILS SHALL REMAIN UNCOVERED AND/OR UNSTABILIZED FOR NO MORE THAN 2 DAYS FROM OCTOBER 1 <br />THROUGH APRIL 30, AND FOR NO MORE THAN 7 DAYS FROM MAY 1 THROUGH SEPTEMBER 30. <br />13. CONSTRUCTION IS ANTICIPATED TO TAKE 1 TOTAL WEEK TO COMPLETE.
